I am auditing a record now, where the physician just wrote "greater than 30 minutes". There is no actual time listed. It is a discharge, coded 99239. Should I downcode it to 99238?
 
I would submit the claim as is. Ideally, the physician should have documented the time more exactly, but it is still a 99239.
