Hello,

I was reviewing some of the questions out of the old 2007 CPC exam study guide book about E&M codes. they gave a scenario of a patient who came into ER for a fall and final dx was fracture. It states that the physician documented a detailed history and exam and moderate mdm. The answer sheet says this should be a level 99282? Is this an error in the book? I thought detailed, detailed and moderate was a level 4? what I am missing?
