99396 bundling 81002

When billing for a preventative visit we are using 99396 (or other age appropriate code), 36415, and 81002. The 81002 always denies as "inclusive". Is there a modifier we should be attaching to this in order to get paid?
 
Have you checked the insurance plan's medical policies to see if they expect you to do a urine test during a preventive visit and if they consider it included?
