do we need adequate supporting documentation for above series of codes. Please clarify. if needed what are the specific requirements.

You need supporting documentation for EVERY code you bill out. There would never be any instance where a code would be able to be billed if it wasn't documented.

99401-99412 are preventive medicine counseling codes. Each code is topic-specific. In your CPT book, read the paragraphs underneath the section, and for each code to get the specifics about what is required.
CMS has some great preventive-medicine tools, including MLM articles for preventive counseling, using the 5-A approach. Medicare requires you bill those with G codes, but the documentation guidelines translate well to these codes.

These codes are time-specific, so time should also be documented.
