Preventive medicine counseling and/or risk factor reduction intervention(s) provided to individuals in a group setting (separate procedure); approximately 30 minutes. I have a provider that is documenting 11-20 mins on her notes. Would you query the provider or go ahead and bill the charge?
 
The midpoint must be passed to report a time-based code unless otherwise instructed. The documentation does not support that 16 minutes or more was spent providing the service. I would definitely query the physician to find out why the actual time of service was not documented and why 11-20 was documented (e.g.,, Did some patients receive 11 minutes of service and others 20?).
