If the provider clearly states it's a phone call but the duration of the call was not documented. Can you bill the lowest level such as 99441? or would this be a non-billable service?
 
The time is required for 99441-99443. You cannot default to 99441 because if it was < 5 minutes, it is not billable.
I would query the provider on this. If the provider does not know/cannot recall, then nonbillable with education to document time in the future.
