Hello,

We have been denied for billing 99460 and 99477 together. It seems these two cannot be billed together even if a modifier 25 is attached. However, I have seen periodicals that says these two codes can be billed together.

My question: Does anyone have some reference(s) to support either opinion?

I have checked around and it seems every explanation for either are vague.
