Hi,

First time a provider used 99483 with 99350 for a home visit, is it okay? Is R41.0 a valid diagnosis for 99483? Do I need any modifiers?

Provider cannot use both these CPT codes together. It seems CPT 99483 cognitive assessment might be viable but depends on setting & documentation. R41.0 sign but what can be more of reason pt suffers with cognitive memory or confusion? It is related to a .past illness or related to chronic condition? Better dx might be G31.84 or F06.8 but add if pt has another chronic illness. Is the pt quite older can add senile R41.81 and add dx I60-I67 or dx I69 if a sequalae of CV condition. Also if alcoholic or drug addict or suffers wiht heart disease or blind could be more reasons too
