Is anyone billing these two Transitional Care Management Codes after a patient
is discharged from OP or IP facility? We see patients in the Skilled Nursing & Rehab
Facilities. We discharge patients who don't usually have a Primary care physician.

I just started billing for the transitional codes. Dont have anything to report about them yet.

I have billed these codes to my local medicare I am getting the denial of timeframe required not sure what that means? Can anyone help Thanks my claims do have there admit and dc dates on them
