Wiki 99495 & 99496 TCM Codes New for 2013

Is anyone billing these two Transitional Care Management Codes after a patient
is discharged from OP or IP facility? We see patients in the Skilled Nursing & Rehab
Facilities. We discharge patients who don't usually have a Primary care physician.

I have billed these codes to my local medicare I am getting the denial of timeframe required not sure what that means? Can anyone help Thanks my claims do have there admit and dc dates on them

Are you billing date of service 30th day. Start with discharge day as day 1 and bill DOS day 30.

bill these codes on day 30 after discharge. Initial fce to face not reported seperately. it is considered part of this code.

I've got some of these coming back denied for the same reason using D/C date as day one and then counting 29 days. Anybody else?
