i'm trying to locate a reliable source for the time constraints on these codes. specifically i'm looking for the limits on 99498. on CMS's website they do set the limit for 99497 at 16 mins minimum. my issue is that they do not do that for 99498 but i just watched 2 webinars that say you must use the 16 min rule for 99498 also. does anybody know where that rule can be found in writing? a nationally recognized resource would be nice rather than somebody's opinion or interpretation of these codes
 
Please see the CMS fact sheet MLN909289. There is a table on page 4 showing that CMS applies the CPT midpoint rule.
