I have a question about a physician billing 99204 - 25 and 99499 for a special report to determine an Emergency Medical Condition (EMC).
The physician does a complete history, physical and exam and also addresses the EMC under impression. Physician performs injections and prescribes medication and
discusses addiction, substance abuse and medication tolerance, stating that 40+ minutes were spent with the patient that involved physical, ortho/neuro exam, review of medical records and thorough discussion of findings and options.
Charges were: 99204 - 25, 20553, 99499* (EMC Report), 94760 and charges for the medication/injections.

Of question is specifically the 99499 - Since the doctor addressed EMC w/in the E/M and included in impression, does a pre-formed EMC report that has standardized wording (not specific to the patient - other than ICD-10 codes) suffice for billing separate for same. The standardized form states the physician spent upt to 15 minutes face to face educating the patient about EMC and Florida Statutes and the time taken was not part of the exam.
