Wiki A new patient and old problem

Hi, I am taking the E/M test next week and have a question about problem points in MDM. If a new patient has an established problem like stable hypertension would this problem get 3 points for "new problem with no workup" or 1 point for "established problem stable or improving". Is a "new problem" new to the Dr. or new to the patient ?
