Wiki A/P Addressing of a Dx

So I was wondering. We get dinged no our audits if we bill a diagnosis that is not "addressed" in the A/P. If this has been noted elsewhere in the
notes is it ok to bill the dx? This comes from our compliance department.

Physical exam shows: "diffuse thinning of hair with visible scalp on sides and crown".

A/P States: Alopecia with nothing as to anything done just that the patient has it.

A/P: Alopecia (L65.9)
