Wiki A9502 with 93015 and 78452 - I keep getting billing error stating

I keep getting billing error stating need medical documentation when A9502, 36000 are billed with 93015 and 78452. Medicare denial states that need medical documentation. I have sent medical documentation and it still is denying. Do I need to send invoice for these, If so, my invoice has several patients names on it. Do I need to black out names that aren't the one I am submitting or just send all of the invoice with the different patients names. Or is there something else I need to do to get these paid. Please help me this is very frustrating I submit the total mCi of Tc99m on the claims and still get denials.
