Hello,

I posted this question once before but unfortunately didn't get any responses so I'll try again...Documentation states AAA s/p repair, would this still code as active or would it be history of? Also, would it be appropriate to use Z95.828 (Presence of other vascular implants and grafts) in addition to the active or history code? The documentation does not state how it was repaired.
