I just started work in a family practice clinic, and I've noticed on the PT/INR claims they are using the QW modifier for CLIA waived on 85610, but getting an ABN signed.....is this correct? The PT/INR should be covered based on the dx I would think, which is the long term coag use and HX of DVT. What constitutes the DVT being history instead of it being current? If it happened 3 yrs ago its history vs 3 mo ago is ? EEKS, help please.
