A very technical ABN question.
Medicare is now saying as of 1/1/13, that if you do more than three visual fields on a patient, one needs to get an ABN for #4.

Does this apply by NPI #, by tax ID # (meaning if I did 3 fields and glaucoma did a fourth on the same patient, does this apply?)

Please help.
