Absence of organ category Z90

Is a category Z90 code, coded for the encounter the organ is removed or is just the reason for removal coded?
 
Since this is a "history of" code I would not use it for the surgery (since something that is currently happening isn't part of your history yet) but I would use it for any visits after the fact.
