Wiki Accidental IUD removal???

IUD was accidentally removed during another procedure. It got hooked on a prep sponge. It was replaced. I'm drawing a blank on this. Should this be coded? Is there a modifier that should be attached indicating "accident". I know the codes for IUD insertion, but drawing a blank as to what to put for this?
 
Can an expert help with the question above? We have a similar scenario where IUD was removed while cutting strings. IUD was not replaced at this time. I was not able to find a modifier for this.

Appreciate any help on how to code :)
 
In my opinion, I don't feel this should be separately billed for the re-insertion. It was the provider's error. Similar to when a provider accidentally lacerates an organ during surgery, you don't separately code the repair.

Open to other's opinions and experiences!
