Wiki Accidental Stick by needle coding question

We have a patient that was stuck by a needle while working in healthcare. The doctor coded a cause of injury (W46.0XXA), but I am wondering what to code for the actual injury. The provider could not find any puncture wound, nor "any sign of discoloration or lesion" by the time the patient arrived at the clinic so I don't feel as though I could code an open wound or puncture wound for this.
Thoughts?

When I code for ER. I code T7589XA and W460XXA, W461XXA is contaminated.
