Wiki ACDF through separate incisions

My physician performed an ACDF at C3-4 and then, through a separate incision, another one at C5-6 and C6-7. My question is, do I code this as 22551 and 22552 x 2? Or 22551 and 22551-59 with 22552 as there was a separate incision and one inter-space was skipped? Any advice is appreciated!
 
If you are going to count this was one continuous fusion then you will need to count your instrumentation the same. The MUE for 22845 is 1 so you wont get paid for 2 separate plates. I have been battleing this issue for almost a year and finally recieved info from a conference to bill this as 22846 and I was paid. You will have to count from the top of the first fusion to the bottom of the last.
