Has anyone billed 17360 ("chemical exfoliation for acne (eg, acne paste, acid))? If so:

Is it payable by insurance?
What kind of "acne paste" or "acid" do you use? (Is this the same as a glycolic peel?)
Can it be performed by an esthetician (with a doctor in the office to bill incident to?)
 
you usually need to get prior authorization first. insurances like FIDELIS Care are ridiculous. They want the patient to try accutane first, if that fails they'll cover the chemical exfoliation code. It would cost them a hell lot more to pay for patients course of accutane than a single chemical exfoliation so kinda dumb on their part. Also they rather the patient suffer through the side effects of accutane =T, I just dont get it.
