I have a provider who is challenging me that he should get additional wRVU value for using modifier 22, I can't find anything on CMS website, and I don't think payors reimburse additionally for this. I think it is just documenting that it took you longer. Any of you experts know any different?
 
I do not know where to find the info for this on CMS but I use modifier 22 quite a bit Medicare will almost alway pay more when this modifier is attached. Records must be sent for them to determine. I could be related to time, patients complex anatomy. A physician here used to dictate in the op note I believe this case should have a 22 modifier and he would list the reasons why. Never had any problem with Medicare. It's the commercial insurances that we come into issues with.
