Wiki Additional Sick Visit?

Hi All,
We have a provider who charged an additional Sick Visit code for the following issues and please advise if it correct?
1.) Medication Refills for chronic illnesses
2.) School Physical Exam or Forms
3.) Pre-employment Physical Exam
4.) Dx. Constipation did not give Rx. such as, Colace but discussed high fiber diet and to increase water intake
5.) Dx. Leg swelling did not give Rx. ONLY instructions to discontinue use of Elastic Bandages

Sounds like a regular preventive visit and not a problem focused visit (E/M). What was billed?

Agreed; based on the above, I would not add an E/M to the preventive visit. If a new medication was added for the acute problems, you would be able to do a low level E/M.
