We had a patient here today for a Saphnelo infusion. As soon as the nurse put in the IV and started the bag the patient's veins blew. The patient did not receive any medication due to the pressure of the IV start blowing out the vein. The patient declined trying to start the IV again. My question is would we be able to bill the 96413 code due to the IV being started? The way I'm reading the description in the CPT book we should be able to. Thanks!
 
I would, with a reduced services modifier since the infusion wasn't completed. The nurse should have input a note (we call it a Treatment Event ) to back that up, if it's not normally in the MAR.
