Wiki Admit and discharge on the same day

Question.
If a doc discharges a pt from the hospital and admits the same pt to the Neurology Rehab on the same day, how do I code that. I already coded the discharge and now I have to figure out how to code the admit. Is there another POS code I can use?

Have you looked at 99304-99306?

I thought of that but it's not a nursing facility. It's Neurology Rehab in the same hospital.
