Would the Admit/Discharge Same Day code set be appropriate in the scenerio below:

Pt admitted on 5/26. However the provider does not physically see the patient until 5/27. He/She performs their initial assessment (H&P) and then makes the decision to discharge either then or later in the day on 5/27. Pt has been in the hospital for more than 8 hours.

you should be able to change 99234-99236, however be mindful of the insurance guidelines on whether it is inpatient or outpatient.
 
Yes, you would use the same days admission/discharge codes. The date is based on when the MD saw the patient, not the day of admission in the hospital's system.
