Wiki admit to nursing facility/help

when a pt comes in to physician office for visit (UTI, fatigue, dementia) and physician admits pt to nursing facility can he charge an E/M for the visit and also charge for the nursing home admit or is it the same guidelines as when a patient is admitted to the hospital you charge for the hospital admit and not the office visit.

Thanks for your quick reply. Would you use Diag. code V70.3 for the admit
 
You would use whatever chronic conditions the patient has that necessitate thier being in the nursing home. The V70.3 would be used if you were billing a visit for the purpose of them going into the nursing home- kind of like school physical but a nursing home physical.
