Aetna denials - recent

Is anyone else experiencing an insane amount of denials and other weird stuff from Aetna and Aetna MedAdvantage plans? For example, they started off downcoding many of our E&M codes. Then started denying anything billed with testing (99213 + 92250). Now they are denying a routine eye exam (paying the refraction but denying the 92014). I am losing my mind over this!
