Wiki Aetna denying cervical cages

Has anyone read the new policy update, handed down by Aetna, that they will respond with non-approval for payment for cervical interbody cages, such as PEEK? Has anyone had any luck with letter of medical necessity or Peer-to-Peer reviews? What was your basis for appeal?
