Good morning!

I am new to Podiatry coding, and have a question about how to code the Affinity membranes.
I am trying to figure this out by going back and looking at previous coders work.
INITIAL WOUND: 2/15 is 3 x 4 x .2 cm

THIS VISIT: 4/26 wound size is 1.2 x .5 x .2 cm

The only documentation regarding application at this visit: "Amniotic applied per manufacturer protocol without waste to plantar left foot."

The previous coder has Q4159 x 7. (with the prep 15275) But I guess I don't understand where she has come up with this. The website mentions "product size, not wound size", but documentation is scarce with the provider.

Could someone guide me in the right direction to code these?
Does the provider need to describe the amount of product used?
 
Q4159 is billed with 7 units if the whole product was used on the wound. The provider should state the amount of the graft that was used and the amount that was wasted if any was.
 
The provider needs to document not only the name of the product but the size and serial number or lot number. To properly code, you will need to know the size of the product. Most come in multiple sizes, so you need to ask what size was used on this wound. The larger wound most likely had the 2.5cmx2.5cm product used. The provider also needs to document if all of the product was used and if not, how much was wasted. Some are afraid to do this thinking they wont get paid for the wastage...they will get paid. Line one would have the units used, lets say this was a 7 unit piece and they used 5 units, that goes on the first line and 2 units on the 2nd line with a JW modifier. Be sure to code the application as well.
