In ICD9 we had a code for allergy desensitization V07.1 but in ICD10 if you do the cross walks it gives you Z51.89 Encounter for other specified aftercare and Z78.9 Other specified health status. Neither one of those codes are specific. Has anyone else ran across this problem and what do you use in ICD10?
 
Medicare is denying that code as not medically necessary

Medicare is denying that also. I have not idea how to bill an allergy shot. They have a list of "approved codes" on the LCD, but mixed vespids, ants are not on the list. They have spiders, snakes and poisonous animals. Does anyone have any idea how to bill an allergy injection under ICD-10 for someone who is allergic to bees?

There is a Coding Clinic for ICD-10-CM, Third Qtr. that states "There is no ICD-10-CM code to reflect an admission for desensitization. Assign the codes for the underlying condition for which the medication requiring desensitization is to be given (example, assign codes for respiratory condition and infectious agent that requires Bactrim) followed by status code Z88.2, Allergy status to sulfonamides, for this example.

Question was "What is the correct code assignment, when a patient is admitted for desensitization to a medication? For example, a patient with chronic respiratory condition is currently symptomatic and recent sputum was found to be growing stenotrophomonas which is sensitive to Bactrim. Since the patient experienced angioedema due to prior exposure to Bactrim, he was admitted for desensitization to Bactrim."

That question addresses a patient that is currently symptomatic. You cannot assign a symptomatic diagnosis to an asymptomatic patient. Starting Saturday October 1 a new code goes into effect for allergy desensitization. Z51.6
