Wiki AMA - Appendix H???

Hello,

Was wondering why did AMA skip H from the appendix section in the CPT book? Is there a reasoning or history behind it?

Appendix H

It actually hasn't been in the last few years editions. The AMA 2015 CPT edition has a notation that the appendix is dynamic and rapidly expanding and now only available online. www.ama-assn.org/go/cpt
