I know that the provider gets credit for reviewing notes from external sources, but do they earn any credit from reviewing prior notes within their practice? And does an extensive HPI coming from the patient during the encounter count for anything?
 
Reviewing your own practice's notes does not count toward data. Since history and exam are not elements of leveling the visit for outpt E/M, again, it does not count.
If you are going to be billing based on time, the time spent by physician/NPP on these activities same day as encounter does count toward total time.
