There is no modifier needed. You either bill the claim with the PA's provider number, or bill the claim "incident-to" the physician's services.

Assistant at surgery
If the PA is an assistant at surgery you would use the AS modifier.
If the PA acted alone you can bill E/M or procedures within the PA's scope of practice under the PA's name without any modifier.

Opps.. she's right! I missed that one. My PA's just do office based E/M and simple office procedures.
