I work for a separate pathology laboratory that was created as a sidebranch of the university's hospital. I code the pathology that is sent to us from "our" hospital, and from other hospitals and clinics all over the U.S.

I am a compliance specialist who works for the hospital. Technical billing is done by the hospital. Professional billing is done by an affiliated physicians' group. There are stand-alone laboratories that do both in house. I tend not to trust farming out billing services to third or fourth parties since they tend not to have a good handle on day-to-day operations and practice, and pathology is a specialized niche in the coding world where coders really need to be on top of their game with few published resources.

I do some consulting work and find in-house billers who are intimate with the standards and practice of a particular pathology lab to be the rule.
