Wiki Anesthesia coding for complication during procedure

I am still new to Anesthesia coding and have a question when it comes to complication that take place during a procedure. I understand you code for the intended procedure but does this apply to anesthesia crosswalk coding since the complication sometimes requires a lot of additional work for the Anesthesiologist? I have a case for Laser lead extraction where there was and injury to the superior vena cava which required emergent sternotomy for cardiopulmonary bypass and hypothermic circulatory arrest which the ASA would be 00560 25+TM but if I code only the scheduled procedure the ASA is 00520 6+ TM. Can anyone help me understand this or know of any written guidelines regarding the Anesthesia coding in these types of cases? I really would appreciate any assistance or guidelines regarding this.
