What anesthesia CPT would you use to code the following, and why? 00190? 00192?

OPERATION PERFORMED:
1. Bilateral mandibular sagittal split rotational setback osteotomy with
*** bicortical screw fixation.
2. Removal of previous hardware (2 small screws) placed in the left maxillo-
*** alveolar cleft area for stabilization of a guided tissue regenerative
*** membrane in 2003.
3. After debridement of the residual alveolar cleft defect, re-grafting of
*** the area with 0.5 cc of Puros allogenic bone.
 
Code each procedure & crosswalk each code using the ASA Crosswalk manual. Use the anesthesia CPT code with the highest number of units.
