We are a gastro practice that use only CRNA for our colon and egd that we do in our office endo suite. For measure #426 I think that we need to bill G9656, G9657 and either G9655 or G9658. Our consulting firm is telling us that we need to bill only G9656 and either G9655 or G9658. I think 3 codes and they say 2 codes. Can anyone let me know how they billed this measure for 2016. we use pos 11.

Patient transferred during anesthetic or to ICU - G9657 - no other code to report.

thanks for the response but I am still confused as to whether we need to submit G9655, G9656 and G9657 or just G9655 and G9656 for 2016. I am fine with what to submit for 2017. We have an office setting so we never transfer care or to the ICU. If anyone could please help that we would be great. Anesthesia was new to us last year and we would like to avoid a medicare penalty.

you have to just follow down the line of questions

was patient transferred directly to pacu? If yes g9656
if no no code at all
and

was hand off/check list elements used? If yes g9655
if no g9658

this is how i understand it, hope this helps
 
If you do not transfer a patient to ICU, you would never use G9657. The only codes you would be submitting for 2016 are G9656 with G9655 (hand off used) or G9656 with G9658 (hand off not used).
