For the PTCA use 35475, stent would be 37205, to do both in same vessel must be medically necessary. Catheter placement is 36215 and you may have S & I codes 75960-26 & 75962-26 if documented.
I am assuming this was done percutaneously.

need a little more info, did the stent cover the subclavian, and extensions, proximal or distal and this done by cut down or open
