Wiki Annual or Diagnostic Labs?

A pateint is seen at her primary care physician's office for her annual well visit because she got a letter in the mail from UHC instructing her to. She is feeling fine, no complaints. She was diagnosed with hypothyroidism many years ago, but has been off of her thyroid medicine for 10 years and has felt fine. Since her TSH hasn't been checked in 10 years the doctor decided to get it checked. If the doctor writes on the lab slip, "Annual Labs, History of Hypothyroidism" will this be a annual or diagnostic???
