Annual Well Visit and Annual Preventative Visit for Medicare Advantage Plans

I was posed a question from one of the offices I work with about billing of the Medicare well visit G0438 and G0439 along with code 99387 or 99397. From the guidelines I see you can't bill both the Medicare well visit G0438 or G0439 along with the preventative visit 99387 or 99397? This is for a Medicare advantage plan claim. The only thing I see you can bill two visit codes would be the annual well visit G0438 or G0439 along with E/M code 99212 - 99214 if a medical diagnosis/acute diagnosis is discussed at the time of the visit. At that time, you would bill the 99212-99214 with modifier "25". Am I on the right page? Thank you.
