When coding a AWV and the doctor orders labs to do in the next 6 months for a chronic condition or refills a prescription for a chronic condition, how is everyone coding these?

We were supplied with a grid sheet from aafp.org that helps determine when to bill only Wellness, Wellness and E & M or just E & M code. There is also one for Preventative vs E & M.
