Has anyone heard starting in 2012 Medicare patients will be required to fill out a Health Risk Assessment form? If so does anyone know where we can find these forms? I have not been able to find any information about such a form on the Medicare website.

Most Medicare Advantage plans will have the forms since it is one of the ways they get reimbursed. Check with one of the MA plans you particiapte with. We usually have the provider fill out the form not the patient. I have not heard of one that the patient needs to fill out.
