Can a RN bill for the actual injection with individual NPI? When billing injections, does it require a supervising and/or ordering physician?
 
I would advise against doing this. It's unusual for an RN to have their own NPI, but even if they do, most payers will not enroll or credential an RN as a provider, so if you bill with this, it will likely be rejected. Also, you're correct, there are very limited circumstances in which an RN can perform services that are not ordered and supervised by a physician. Medicare requires all services that are for the diagnosis and treatment of an illness or injury to be incident to a physician's plan of care in order to be eligible for coverage. The RN's license also probably requires a certain level of supervision. Performing and billing for unsupervised services could be risky and potentially noncompliant.
