Lumbar arthrodesis is performed using a pre-sacral interbody technique including instrumentation, imaging, and discectomy to prepare the interspace. The pre-sacral interbody technique, also referred to as trans-sacral or paracoccygeal, uses a minimally invasive percutaneous approach to the anterior portion of the disc space. The patient is placed in a prone position and a small incision is made at the level of the coccyx slightly lateral to the midline. A trocar is advanced anterior to the sacrum under fluoroscopic guidance. A small presacral channel is created in the disc space. The two adjacent vertebral bodies are distracted, and the disc interspace is prepared by removing the intervertebral disc material and the cartilage from the end plates of the adjacent vertebrae. Separately reportable bone allografts (donor) or autografts are obtained. Spinal fusion (arthrodesis) is accomplished by packing the bone graft material into the disc space. The body of the superior vertebra is drilled, and an anterior fixation rod or screw is implanted. Use code 0195T for arthrodesis of a single lumbar interspace and 0196T for each additional interspace treated.
