Anthem Medicaid PA Denials

Not really coding related but, we have been getting a lot of PA denials from Anthem Medicaid. Anthem Commercial and Anthem Medicare are being processed just fine. The most common code we bill are J0717, J1745, J1602, J0129, J3247, J0491, J0490 and Q5104. We do buy and bill which is what we indicate on the PA request and they usually go to CarelonRx or CoverMyMeds. Is anyone else having PA issues with Anthem Medicaid? How are you guys doing your PA's?
