Preventive visits states documentation includes counseling/anticipatory guidance/ risk factor reduction interventions. What specific physician documentation should there be to indicate anticipatory guidance? Would this qualify?

Recommendations for this patient's age were placed in the context of the personal and family history.
Chronic conditions were reviewed and new concerns addressed.

Colon cancer (age): 01/18/2023
Prostate cancer screening (age, grade C recommendation-shared decision making): Done
AAA screening (age, once in a lifetime for past smokers): No
Flu vaccine-Recommend: Annually
Tdap-Last: Today. Next Due: 10 years
Shingrix (age): Complete
Prevnar 13: Only if high risk
Pneumovax 23 (age): Age 65
Hepatitis C screening: Ordered
Low-dose chest CT (age w/20 pack year history current smoker or quit within last 15 years: N/A

Anticipatory guidance is going to be patient-specific. It refers to the patient/provider discussions related to any preventive or risk-reduction advice as relevant to the patient's current lifestyle. For example, I am a stained glass artist and my PCP recommends I be tested for serum lead periodically.
