We are billing for the 99364 and receiving rejections, from PH and BCBS. Is anyone having luck getting them paid and if so how are you billing them?

You should look at the timing of the encounter and the dx codes should be the Z51.81 followed by the Z79.01.

Date spans always mess me up but this code is 1 unit per 90 days. Minimum of 3 INR must be performed within the period to report. This code is for subsequent to the first 90 days of treatment.
