Very frequently, at the hospital I'm working for, the physician will list "Never a smoker" in the Assessment of the H&P. The diagnosis code will follow the written dx on their office notes. What they are using is V49.89. I also see this this code used under Personal/Social HX. My question is has anyone seen this before? I wouldn't normally pick up "non-smoker". I have been doing so if listed under the Assessment. Any thoughts about this? I just don't see why I would pick it up.

Thanks,
Mary Dillie, CPC-H
